Membership and prompt contributions to the Philippine Health Insurance Corporation is mandatory for all government and private sector employers – even those that employ house helpers (Kasambahays) and family drivers. This is stipulated in PhilHealth Circular No. 016-2015 that covers the registration of household employers and their employees (kasambahays) through the Kasambahay Unified Registration System.
The Kasambahay Unified Registration System was established in 2013 to provide household employers and Kasambahays a one-stop shop registration process for all mandatory government memberships such as SSS, Pag-IBIG, and PhilHealth. This means that a household employer and their employed helpers (yayas, cooks, etc.) may register and get their membership numbers for all three government agencies without the need to physically go to each of their offices. All they must do is choose one of any of the said offices, go to the nearest servicing branch, and apply for membership and enrollment of the Kasambahay for all three agencies in one go.
How to register as an Employer:
- Secure a copy of the Household Employer Unified Report Form or PPS-HEUR1 (for the employer). This may be downloaded online or request for a blank copy at the service office where the application will be submitted.
- Fill-out the forms with complete and accurate information and submit them to the chosen service office of either SSS, Pag-IBIG, or PhilHealth.
- Wait for the generation and issuance of unique permanent PhilHealth, SSS, and Pag-IBIG Fund employer numbers.
How to enroll a Kasambahay employee in PhilHealth:

The employer must fill out a Household Employer Unified Report Form (PPS-HEUR2). Submit the accomplished copy to the service office of PhilHealth.
- If the Kasambahay being enrolled has not yet registered to SSS and or Pag-IBIG, the employer must fill also out a Kasambahay Unified Registration Form (PPS-KURF). This will be attached to the PPS-HEUR2 form.

Submit the accomplished forms along with the following ID requirements (of the Kasambahay):

PRIMARY DOCUMENTS – any one of the following; bring original and photocopy
- PSA birth certificate
- Baptismal certificate
- Driver’s license
- Passport
- PRC card o Seaman’s Book (para sa OFW applicants)

SECONDARY DOCUMENTS – any two of the following; bring original and photocopy
- ATM card
- Bank Account Passbook
- PSA birth certificate of children
- Baptismal certificate of children
- PSA marriage certificate
- NBI Clearance
- Police Clearance
- Postal ID Card
- Voter’s ID/Affidavit
- School Records
- ID card issued by LGUs

Other Reminders:
- In case the Kasambahay is unable to present any of the above-mentioned IDs and documents, the application will still be processed and they will still be issued their PhilHealth, SSS, and Pag-IBIG numbers.
- Their membership status will be tagged as temporary until they are able to present the required documents. Their membership ID numbers will only be used for the payment of their contributions and employee reporting.
- Benefits and privileges from any of the three government offices will only be provided to those with permanent membership status.
- The Unified Registration is not applicable to Family Drivers. The enrollment of a Family Driver must be done in all three government offices (SSS, Pag-IBIG, and PhilHealth).
How much is Kasambahay’s PhilHealth contribution?
The fixed starting premium for PhilHealth contributions is Php 400 per month for employees whose monthly wage is Php 10,000 and below. The employer may choose to pay for this in full or split the amount with the Kasambahay. Below is an illustration of the contribution breakdown based on salary brackets

Important Reminders:
- Contributions must be paid on or before the 25th calendar day of the month.
- Advance contribution payments are welcome (to avoid missed payments). The Kasambahay’s PhilHealth contributions may be paid in advance up to two calendar years. This is most applicable if the employer is abroad or travels a lot.
- PhilHealth coverage starts from the time of enrollment and payment of the premium contributions.
House helpers not only get housework done for us, but their presence also allows us to do more for ourselves and our families. The time we would otherwise spend cleaning our homes, doing our laundry, or cooking our food can now be invested in other important matters such as helping our kids with homework, working on a side business, or simply having quality time with our families. As responsible employers, it is but right that we ensure that our Kasambahays’ health and well-being are taken care of. By paying their complete PhilHealth contributions on time, we are not only fulfilling our legal obligation but are also showing them how much we appreciate all the things they do for us.
Make sure your Kasambahays are properly enrolled in PhilHealth. Help them secure an official PhilHealth ID as well so they would have a valid government-issued ID to use when claiming their PhilHealth benefits.
